Services:        Maintenance of medical records and medical information in accordance with Federal, State regulations and hospital policy.
 
                        Release of information services are provided in keeping with the applicable privacy regulations. To assist the department in meeting your release of information needs please note the following:
                       
·        Valid patient authorizations are required.
·        A minimum of 72 hours notice is requested.
·        Fees for reproduction services are in accordance state regulations and revised annually. These are published on Pennsylvania’s state website www.state.pa.us

Assistance in obtaining a birth certificate: For persons born in Pennsylvania, certificates are issued by the Division of Vital Records in New Castle. You may contact vital records via phone at 724-656-3100 or fax 724-656-8951. If you were born in a state other than Pennsylvania you will need to contact the appropriate stated agency for the corresponding state. An internet alternative is www.vitalcheck.com. There is a fee for research and reproduction which varies from state to state. To expedite your request the following information should be available:
           
·        The full name of the person for whom the certification is requested.
·        The father’s name
·        The mothers name , including maiden name
·        Date of birth
·        Place of birth
·        Return address
 
 
 
Assistance in identifying your blood type: Blood type is recorded in the record only if a blood typing was ordered by your doctor. This is typically only done in cases were transfusions are required or anticipated in preparation for a major surgical procedure (i.e. orthopedic, hip replacement). If you have donated blood you have been given a card with your blood type.
